‘Call for Nominations’: SRS Champion Award; OT Preceptor/PT/SLP Clinical Instructor; and, SRS Distinguished Alumni Awards
It is time to acknowledge and celebrate the contributions people have made to the School of Rehabilitation Science! We are calling for nominations for the following three (3) awards:
| SRS CHAMPION AWARD | FOR WHO: | Organizations/agencies/clinics, Clinicians, Community members (people employed with the university are not eligible) |
| CONTACT DETAILS: | Include email, address, and telephone number. | |
| RECOGNITION: | Certificate | |
| CRITERIA: | Consistent/significant/long-standing contribution to the SRS in the form of research, and/or education. | |
| MECHANISM: | Letters of nomination should include rationale. Self-nominations are not accepted | |
| OT PRECEPTOR
PT CLINICAL INSTRUCTOR SLP CLINICAL INSTRUCTOR |
FOR WHO: | This award seeks to recognize the commitment to excellence and outstanding contributions to the educational development of future occupational, physical therapists, and Masters of speech-language pathologists at the SRS, McMaster University. |
| ELIGIBILITY: | 1. Must have been a clinical preceptor/clinical instructor for a minimum of three (3) years for McMaster University.
2. Must have not received the award within the last five (5) years; attention will be given to previous nominations. |

| CRITERIA: | 1. Will demonstrate high standards of professionalism.
2. Will demonstrate commitment to students as a professional mentor. 3. Will demonstrate a spirit of cooperation with the School of Rehabilitation Science. |

| RECOGNITION: | Certificate plus $100.00 Amazon Gift Card (per discipline) | |
| MECHANISM: | Complete the nomination form (attached). Self-nominations are not accepted. | |
| SRS DISTINGUISHED ALUMNI AWARDS | FOR WHO: | This award acknowledges graduates from McMaster University, from the Occupational Therapy, Physiotherapy, Speech-Language Pathology, Rehabilitation Science (MSc/PhD), and Master of Health Management programs. There will be one award per program each year, and if there are no nominations, no award will be given. |
| CRITERIA: | The award winner will inspire a passion for personal growth, development, and life-long learning. The award will honour and recognize alumni who are respected and admired for their leadership, achievements, and contributions in any of the following area(s): Scholarly activity; Education; Clinical Practice; and, Community-at-Large. | |
| RECOGNITION: | Recipients will have their names added to the wall plaque, located outside of the IAHS, Room 403, along with a trophy/plaque. Recipients will be asked to give a short speech at the program’s annual convocation celebrations. | |
| MECHANISM: | Complete the nomination package (attached). Self-nominations are not accepted. |

SRS Awards will be presented after the School Council meeting on Wednesday, June 21, 2023, at 12:15 pm via zoom.
